The Comox Military Family Resource Centre (MFRC) is committed to protecting your privacy. The personal information collected on this form is gathered in accordance with BC's Personal Information Protection Act (PIPA) and is collected under the authority of the National Defence Act to administer our local community programs, workshops, and family support services. 

This information will only be used to process your registration, manage program logistics, ensure participant safety, and facilitate anonymous, aggregated reporting to Military Family Services (MFS). Your information will not be disclosed to external third parties without your explicit consent unless required by law. 

By submitting this form, you consent to the Comox MFRC collecting, using, and disclosing your personal information for these stated purposes. You may withdraw your consent at any time, though doing so may limit your ability to participate in certain programs.
